I haven't had this problem with a phone, but I have with my video camera when trying to upload video to my laptop. Just check the cable and make sure its in all the way. If any of the little contacts come uncontacted it will "disconnect" but it will look like its plugged in.
I have to plug it in, have the laptop recognize it and then just set it on the desk and leave it untouched so it doesn't disconnect.
